I've only ever seen an actual R-rated preview once, the kind with the red screen instead of a green one.
Ironically, it was in front of Bend it Like Beckham - or else it was in front of Spirited Away, I can't actually remember which. I'm pretty sure they were just the previews that the arthouse theatre had on hand, but, uh. Yeah.
