You could in theory call one a "teethbrush", as it is not truly intended to brush a single tooth; but as that is not the accepted term for the item, the proper plural is unambiguously of the "brush" portion.

I wondered because the proper plural of "passerby" is apparently "passersby," although that may have more to do with the form of the word (that you can pluralize brush, a noun, but not by, a particle.)

Some compound words whose latter part can be pluralized behave like "passer(s)by", though. Like "attorneys-general" or "gins and tonic" or "surgeons-general". IIRC, it's because they're hierarchical terms, so it's the base element that gets pluralized, no matter which part of the word it is. In the case of toothbrush, "brush" is the base.
