If a restaurant is going to cook steak in a non-standard way (or if you know your area cooks it differently than others), it needs to say so in the menu. My favorite NoVA restaurant does-- it tends to be on the rare side, so it has a handy guide to what the words mean there (along with "well done- order chicken", which always amuses me), and it's the one place that has never managed to fuck up my steak.
