That's apparently the first time it's ever happened on the show (can't find the comment that confirms this, though). The producers were pretty horrified.

I read that comment too. It was posted by the guy in the episode who was there the night before Ramsey arrived. He ordered the pizza, waited for an hour and half, got fed up and tried to leave only to have Sammy and Amy call the police because he wouldn't pay for the pizza he never got.

I thought it was on the Yelp reviews, but I can't find it buried under all the parody reviews.
