In all honesty, I really thought d'oh was meant to make fun of the way people with cognitive disabilities supposedly sound also? Or is is that "d'oh" and "duh" are not the same?

Erf, not really. And I think there is room out there for words which are meant to say "I'm making a meaningless and hopefully amusing noise because clearly I am not processing life in any way right now."
