AFAIK, "d'oh" and "duh" aren't the same. "D'oh" comes straight from Homer Simpson, and he seems to just use it as the sound equivalent of a forehead slap. I've never heard anything behind why/how they came up with that particular sound for him.
Not that Homer himself doesn't use "duh", as well.
